活动的豁免条款

如果您参加本次活动，则将被视为同意并接受以下条款。
1. 我完全自愿参加为社区健康跑步的活动（5K Run/Walk for Community Health）；
2. 一旦申请被接受，我（我的继承人或代理人）将完全放弃因参与该项活动，可能对我造成的伤害、疾病或其它损失，向承办方、赞助商及志愿者提出诉讼和索赔的权利。
3. 我确定我的身体状况良好，可以在此活动中跑步或行走，并承担参与该活动的所有风险。
4. 我也允许承办方合法、无偿使用我的姓名和照片。
5. 我承认承办方有保留取消活动或更改日期和/或时间的权利，如果取消或更改，报名费将不予退还。

Waiver Statement

If you participate in this event, you will be deemed to agree and accept the following terms.
In consideration of acceptance of my application to participate in the 5K Run/Walk for Community Health, I waive all claims for myself, my heirs, and assignees against OllieTree Foundation (a local not for profit organization), and all sponsors and their respective directors, officers, employees, agents, volunteers, and representatives for injury or illness which may result from my participation. I am in proper physical condition to run or walk in this event and assume all risks associated with my participation. I give permission for the uncompensated use of my name and picture in any written account, broadcast, or telecast of this event for any legitimate purpose. I acknowledge that the event director reserves the right to cancel the event or to change the day and/or time, and in the event of cancellation or change, there is no refund of entry fees.
